Medical Consent

I give permission for the coach to seek emergency medical treatment for my child if necessary, including contacting emergency services (999) when required.

Acknowledgement of Risk and Liability Waiver

I understand that participation in football training and sporting activities involves a risk of injury.

I agree that KONEVYCH FOOTBALL DEVELOPMENT CENTRE LTD, its coaches, and representatives are not liable for injuries, damages, or losses sustained during training sessions or related activities, except in cases of proven negligence.

Rules and Safety Policy

• The child must wear appropriate sports footwear.
• The child must bring water.
• Parents must inform the coach of any changes in the child’s health condition.

Personal Data Protection (GDPR)

Personal data will be stored securely and used exclusively for:

• organising and delivering football training
• ensuring participant safety
• communication regarding sessions, schedule changes, and administrative matters

Data will not be shared with third parties without lawful grounds. I confirm that I have read this information and give consent for the processing of personal data.
